Felt Report Questionnaire Information

Report your experience of a recent earthquake using our questionnaire.

Introduction

We ask you to identify the effects of an earthquake at the location you felt it. If the earthquake has been damaging to your building, you will also be prompted to report the effects on your nearby community, but responding to this stage of the questionnaire is optional.

Your name, email address, and the address of the location where you felt the earthquake are a necessary part of the questionnaire. We use your name and email address to make any follow up correspondence, whilst your location is used in the compilation of all responses for a particular event. We are currently developing web pages which will allow you to see the number and spread of responses to felt earthquakes.

Address matching

We attempt to match your address against Land Information New Zealand's New Zealand Geographic Place Names database. If you live in a rural area, and your homestead name cannot be matched, then use your nearest locality name. If possible, please enter your rural road address, which will be useful in the future when we may be able to match against road names as well as place names.

Common mistakes are to spell a place name incorrectly, or to place too much information on one line. For example, do not enter both suburb and town on one line. For someone feeling the quake in Silverstream, the second line of the address should be just Silverstream not Silverstream Upper Hutt, with Upper Hutt then entered on the third line.

Accuracy of information

It is important that the reports are associated with the correct earthquake, so we would appreciate it if you could ensure the date and time are as accurate as possible. Note that times that are am run from midnight to midday, and times that are pm run from midday to midnight.

Inactivity timeout

If you pause for more than 10 minutes whilst filling in the questionnaire you will lose the information that you have entered and will need to start over.

Browser back buttons

Please do NOT use the back or forward buttons on your browser to switch between pages in the questionnaire. The questionnaire contains buttons (continue or re-submit) to move forward through the questionnaire. Amend buttons on the final review page allow you to go back to a page if necessary.

Problems

If you experience problems completing the questionnaire, please write to the Felt Reporting Officer with a description of the difficulties; attaching a copy of any error messages displayed on your screen will be very useful for troubleshooting. It would also be helpful if you could state the operating system and web browser you use, together with the browser version number.
